The Mental Health Assessment page displays all tabs and fields from a variety of assessment and allows users to enter and save the assessment for individual clients.
To Open the MHA, hover over the Assessments banner, then click on the MHA sub banner.

Another way to access the MHA is to click on the create document drop down and select Mental Health Assessment.

Create and complete an Initial/Annual MHA.
When the MHA first opens it opens to the general tab. This tab displays basic information that generates the majority of the dynamic functionality in the assessment.
The Initial/Annual check boxes determine if the assessment is being completed for the first time for the client or not.
Additional Content Check boxes are determined by which tracks and teams a client is currently enrolled in. These enrollments determine the tabs that are displayed in the assessment.
If the Mothers Project (parenting) check box is checked then the parenting tab in the assessment will appear.
If the Supported Education (Education Barriers and Preferences) check box is checked then the Edu Barriers & Pref tab in the assessment will appear.
If the Supported Employment (Emp Barriers, Emp History, Emp Preferences) check box is checked then the Emp Barriers, Emp History, and Emp Preferences tabs in the assessment will appear.
If the Substance Use/IDDT (Alc/Drug Use Scale, SU Details/Context) check box is checked then the Alc/Drug Use Scale, and SU Details/Context tabs in the assessment will appear.
If the Youth/Young Adult check box is checked then additional fields will appear in tabs through out the assessment.
Current Team Enrollments and Current Track Enrollment text boxes are non-editable and are populated based on which Teams and Tracks that a client is enrolled in.
Text boxes that have a None Reported Checkbox associated with it allow the user to not have to enter information into the required text box if the check box is checked. The application will save text in the text box regardless of if the check box is checked or not. The information will be saved in the checkbox until the document is signed at which point the application will clear the text if the None Reported text box is selected.
